•Misregistration impacts power/cost of registration-based imaging validation studies.•We derive 3 power calculations that model the uncertainty due to registration error.•Formulae relate sample size, registration error and minimum detectable difference.•Formulae predicted sample size, minimum detectable difference and power to within 1%.•We illustrate applying the formulae using a case study modeled on an ongoing study.
