•Most previous clinical decision support systems focus on particular drugs such as warfarin and insulin.•Clinical decision support interventions should be focused on life-threatening events and key organizational safety goals.•Blocking type interventions were effective but there are risks of new types of errors and prescription.•When deploying a new clinical decision support system, it is essential to monitor it to ensure that it operates normally.
