•An adaptive two-level approach for retinal vessel extraction is proposed.•The proposed strategy relies on the novel orientation-aware detector that can efficiently model locally oriented and linearly elongated structures.•Wide vessels and thin vessels are special-varying, and they should be adaptively detected segmented.•Performance of the proposed approach outperforms the most recent methods. |
