•We solved a routing problem arising in the transportation of biomedical samples.•The problem is approached as a multi-trip VRP with time windows.•Real data were provided by the Quebec's Ministry of Health and Social Services.•Desynchronization of arrivals to depot, an important issue in practice, is introduced.•A model and a very efficient heuristic which produces very low gap solutions were proposed. |
