•Preventing erratic behaviors could prevent faults from being introduced or going undetected.•Mismatches between development context and practices could signal erratic behaviors.•Proactive Erratic Behavior Analysis (PEBA) method as a proactive complement for existing RCA methods is developed.•PEBA is resource-friendly, flexible and appropriate for SMEs as well as large organizations.
