•We propose a novel technique for risk evaluation in FMEA.•New triangular Pythagorean fuzzy numbers describe interrelations between risk factors.•Defuzzify triangular Pythagorean fuzzy numbers to calculate risk priority indexes.•Visual description of risk factors in the form of a Pythagorean fuzzy digraph.•Case study of a steam valve system shows the potential applications of the model.
