•Patient safety issues from the use of health IT systems motivated the need hazards detection tools.•We propose a new nine-stage methodological framework for hazards detection in health IT.•The framework includes a requirement for consultation with multidisciplinary domain experts.•The framework provides structure for reducing risks to patient safety.•We also presented two use cases to demonstrate the application of the framework. |
